Unfortunately, we are unable to offer you the ability to complete an on line Medical Questionnaire.  If you feel you have medical conditions that qualify for priority please click below to download a copy which you can complete and post to any of the partners for assessment.

You are reminded that medical priority is only given for severe and permanent disability.  Medical priority is NOT given for reasons such as dampness, problems with neighbours, harassment, pregnancy, nervous debility, state of anxierty, marital problems or illnesses of a temporary nature.
